2. Montebello Crossing – Public Hearing–Continued

Site Plan/ Subdivision, Amended Site Plan

250 Lafayette Avenue, Montebello, NY

Application of Montebello Crossing, LLC, 100 Dutch Hill Road, Suite 340 Orangeburg, New York, for 250 Lafayette Avenue, Montebello, New York.  The applicant is proposing a mixed use development consisting of a 132 unit, 200 bed assisted living facility, a 14,600 square foot CVS pharmacy with drive-through, and a 10,000 square foot office building.  The project will also consist of amending the site plan for Hemion Holdings shopping center to the east of the site.  The property is located at 250 Lafayette Avenue, on the North side of Route 59, approximately 350 feet west of the intersection of Hemion Road in the Village of Montebello, which is designated on the Ramapo Tax Map as Section 55.10, Block 1, Lot 2 in the R59 DD Zone.

3. Loren Ware – Public Hearing–Continued

3-Lot Subdivision and Stream and Wetlands Permit

10 Wilbur Road, Montebello, NY

Application of Loren Ware, 5 Cragmere Road, Suffern, New York, for approval of a Subdivision and Wetlands and Stream Protection Permit entitled “Ware Subdivision.”  The applicant is proposing 3 separate lots each with a single-family dwelling with driveway access from Wilbur Road. The property is located on the South side of Wilbur Road, approximately 700 feet west of the intersection of Haverstraw Road (Route 202) in the Village of Montebello, which is designated on the Ramapo Tax Map as Section 40.20, Block 1, Lot 5 in an RR-50 Zone.

4. Manhattan Beer Distributors—Public Hearing

Re-Approval for Amended Site Plan and Wetlands and Stream Protection Permit

20 Dunnigan Drive, Montebello, NY

Application of Manhattan Beer Distributors, LLC, 955 East 149^th Street, Bronx, New York 10455, for re-Approval of an expired Revised Site Plan entitled “Proposed Expansion for Manhattan Beer Distributors, LLC” originally approved on June 9, 2015 and a Wetlands and Stream Protection Permit.   The proposed construction is for proposed addition of a parking area for trucks, fifteen existing spaces for trucks to be converted to additional parking for employees. Also proposed is a filling station for compressed natural gas to serve vehicles for Manhattan Beer use only and to allow disturbance and construction within 100 feet of a freshwater wetland.  The subject property is located on the northerly side of Dunnigan Drive approximately 1500 feet west of North Airmont Road in the Village of Montebello, which is known and designated on the Ramapo Tax Map as Section 55.07, Block 1, Lot 12 in a PI Zone.
